Observed a Y-valve attached to a mop sink faucet at the time of inspection. The installation of a Y-valve on a faucet with or without an AVB is prohibited and is a violation, because the system is not designed, constructed and installed according to law. Y-value and attached hoses were removed at the time of inspection. Corrective Action: Corrected at time of inspection.

Observed nonfat yogurt held at 51*F and almond milk held at 48-51*F in the reach in fridge at time of inspection. According to PIC, the TCS items were placed in the reach in fridge around 4am, 12 hours prior to inspection time. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.
