Inspector's notes
Observed a chemical dispenser connected to the mop sink equipped with an AVB. Informed PIC that shut-off valves (chemical dispenser) are not approved for continuous installation downstream from atmospheric vacuum breakers. PIC removed the shut-off valve downstream from the AVB at time of inspection. Discussed with PIC that chemical dispensers should have their own dedicated water supply. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Corrective Action: Corrected at time of inspection. PRIORITY VIOLATION-5-203.14 - Priority: Backflow Prevention Device, When Required Addl Notes: Observed RO system installed for a hot and cold-water dispenser without an ASSE 1013 (RPZ) installed upstream. PIC stated that the system was installed about two months prior to inspection. Informed PIC that a backflow device meeting ASSE 1013 standards is required to be installed upstream from the RO system. All RO systems must have an approved reduced pressure zone assembly backflow device meeting ASSE 1013 standards installed upstream from RO system. Provide an approved backflow device prior to re-inspection. All testable backflow devices must be certified. Corrective Action: Correct prior to re-inspection on 6/16/2026.
