E&D Class 2, 10+ seating.Correct the following items prior to your routine inspection:1) Flash off both sides of walk-in cooler to walls with a smooth, non-absorbent, easily cleanable material.2) Finish flooring under water heater so that it is smooth and easy to clean.3) Provide vinyl faced ceiling tiles in restroom.4) Make restroom door self-closing.5) Provide a covered trash receptacle for restroom.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Project nearly complete.Correct the following items:1) Provide hot water to all sinks – water heater not functional during inspection.2) Provide a dedicated water line to chemical dispenser at mop sink.3) Finish plumbing drain of walk-in cooler to floor sink. Drain must terminate above floor sink with a minimum 1” air gap.4) Install a stainless steel splash guard in-between hand sink and prep sink (back room).5) Install a stainless steel splash guard in-between hand sink and prep table (front service area).6) Flash off both sides of walk-in cooler to walls with a smooth, non-absorbent, easily cleanable material.7) Seal walk-in to floor with silicone or caulk.8) Finish flooring and cove base under water heater stand.9) Fill gaps in flooring around clean-outs across from 3-compartment sink.10) Provide vinyl faced ceiling tiles in restroom.11) Make restroom door self-closing.12) Provide a covered trash receptable for restroom.13) Seal all gaps/penetrations in front service area, namely electrical conduit under front service counter.No County legal action will result from this inspection.Report emailed.
