Equipment Inspection. Walked with Leo and Charlie. Fellow PR Senior FC present. Project nearly complete. Establishment is approved to stock and train, but not to serve the public at this time. Do not use hood until approved by Fire Inspector.Correct the following items prior to Final:1) Install a hand sink on outside of bar, next to ice bin. Ensure hand sink has hot and cold water and drains to sewer. If it is installed within 24” of the ice bin, also install a splash guard in-between hand sink and ice bin.2) Re-plumb 3-compartment sink so that it drains to a floor sink with a minimum 1” air gap. Ensure floor sink goes to the grease interceptor. 3) Install white FRP on interior of sushi bar; black FRP is not approved for this area.4) Relocate backflow for soda system so that it is installed no higher than 5’ AFF.5) Install an ASSE 1022 (in-line dual check backflow preventer, such as a Watts SD-3) on all water lines to coffee/tea brewers and to ice machines.6) Plumb waste line from kegs in walk-in cooler to a floor sink with a 1” air gap.7) Complete requirements for Other Effective Means of Barrier Protection. This includes installing fly lights (one in bar, one in sushi bar, two in main kitchen), having fly treatment outside (either bait stations or residual treatment), rodent bait stations inside and outside establishment, bird prevention treatment outside, as well as treatment for crawling insects. Daily logging of inspection of premises (inside and out) for evidence/presence of pests is required (done by a Kona employee) and employee training on pests and closing the continuous openings is also required. Otherwise, install fly fans above all continuous openings.8) Provide sliding lids for all ice bins.9) Provide a cleaning policy for to-go area, as previously discussed over email.10) Provide a clean out cover where missing in to-go area.11) Provide additional lighting in restrooms.12) Seal all gaps/cracks/penetrations. Plumbing Inspection - virtual inspection via FaceTime with Leo. Project appears about 60% complete.Reviewed work done to date, stip letter items. Superintendent had not received stip letter, stip letter will be forwarded with report.Correct the following items:1) Install a hand sink on exterior of bar at ice bin. Hand sink must have hot and cold water and drain to the sewer. Provide soap and paper towel.2) Replace black FRP with white or a light color FRP in sushi bar area, to-go area.3) Either install air curtains at all continuous openings (sliding/rolling doors, windows), or complete Other Effective Means of Barrier Protection Requirements. This will include extensive pest control including for crawling inspects, flying insects rodents, and birds, as well as extensive active managerial control including daily staff monitoring for pests (verified log/checklist) and employee training. Bar must also meet outdoor bar requirements, including providing sliding lids to all ice bins.Ceilings must be light in color - due to virtual inspection, could not fully determine if ceilings in restrooms meet this requirement.
