Final construction inspection for Portillo's Hot DogsConstruction complete and approved. Operator will follow up on:1). Replacing the AVB at the pot filler with a Pressure Vacuum Breaker.2). Adjusting the back door self closer so it falls into lock.3). Adjusting the fly fan so the unit turns off when the door is closed.4). Some of the hand wash sink sensors are finicky, replace if not working dependably. All hand wash sinks are working, but some are slow to react.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Pre-final inspection walk through with construction manager. The following items were discussed:1). Silicone the drip trays at the counter junctures. Two locations drive through and the soda station.2). Rigid plumbing will be installed for both drip trays.3), Install coved base along the front of the dining room soda station. Both sides.4). Check all door sweeps and thresholds and make sure that the doors are insect tight.5). Caulk the underside of the front counter, both sides.6). Provide a fitting lid for the ice bin on the soda station in the dining room.7). Please e-mail test reports for the two backflow valves.8). Zip tie drain lines under the drive through soda station together, also merge with drip tray drain.9). Caulk the hood to the floor, provide a clean beveled caulk line with a durable caulk.10). Install a reduced pressure zone backflow valve on the submerged inlet for the hot wells, install with hub drain.- Test report will need to be provided.11). Caulk the hood batwings at the wall all the way, east side hood.12). Install a hose bib vacuum breaker at the faucet in the riser room.13). Caulk the wall caps for center walls in the dish area where open at the floor base juncture.14). Make sure drive through window is operational.15). Cut back the strip curtains at the walk-in entrances.16). Freezer is missing one cam hole plug.17). Were panels stand open in the walk-in caulk, back across from the refrigeration unit.18). Install a drain for the drain pan in the water heater room.19). Install the condensate line for the water heater vent. 20). Seal wall penetration from water heater room to the exterior.Location is planning on opening on temporary power. Store does not have gas and can not commence with training.
