Observed no hand drying provisions at the hand wash sink located near the 3 compartment sink. The cook re-stocked paper towel dispenser with paper towels. Each handwashing sink must be provided with hand drying provisions at all times. Addl Notes: Observed hand wash sink across from the flat top without hand cleanser. It was discovered that the dispenser was out of batteries but did not have any batteries on site. The cook made it so that soap can be used from the dispenser by pushing the bottle up. Each handwashing sink must be provided with a supply of hand cleaning liquid, powder, or bar soap. Observed raw shelled eggs stored above onions and cut potatoes located in smaller walk-in cooler. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Observed spray nozzle attached to hose on mop sink faucet. The faucet at the mop sink has an AVB, which has been under pressure. Had PIC disconnect spray nozzle from the hose and explained to keep the hose disconnected when not in use at the time of inspection. Discussed with PIC about having the AVB looked at by a plumber since it was under pressure to prevent failure. Instructed PIC to ensure that nozzle is removed every time in between uses. Corrective Action: Corrected at time of inspection.

This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Advisory visit for new business owner.The establishment is not open for business The permit is not approved at this time.Address the following items:(1) (1) Provide an additional hand sink in the area across from the salsa bar/drive through. Hand sink to have both hot and cold water. Ok to run drain line to the floor sink.(2) Provide a reduced pressure backflow assembly (ASSE 1013) on the water line isolating the mister system.(3) Remove all unused chemical dispensers at the mop sink and 3-compartment sink.(4) Provide the missing hot water handle on the food prep sink.(5) Paint the exposed wood located over the mop sink.(6) Install the drain for the salsa bar. Use pvc, routed to the floor sink. Maintain a 1 inch air gap(7) Ensure that both tankless water heaters are functioning.(8) Have all backflow assemblies tested, provide test reports on follow-up inspection.
