Final construction inspection. Construction complete and approved.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. This can be a metal stem thermometer or a thermocouple.5). Provide a thermometer in each refrigerator and freezer so you can monitor the ambient holding temperature in the unit. Make sure to place them so they are easily accessible and not in front of a cooling fan giving the wrong reading.6). Please provide non-latex gloves for handling ready to eat foods and make sure they are not latex.7). Please provide dish wash soap and a sanitizer like bleach for proper dishwashing. 8). Bring all food handlers cards and Certified manager card on site and keep on file for review.9). Provide hand soap and paper towels at every hand wash sink.All other items were in order. Pre-final walk through with owner and the superintendent. The following items were discussed:1). Drain pan drain will need to be led to the mop sink.2). Please caulk the toilet at the floor juncture.3). Cut back the toilet mounting bolts and install caps.4). Provide hot water in the bathroom at the hand wash sink.5). For the RO system. Install a reduced pressure zone backflow valve (ASSE1013) upstream of the RO system. Install with hub drain and lead drain to the floor sink. Install valve between 12 inches and 5 feet from the floor. Valve will require testing (certification) before final inspection.Store will be approved without ceiling with supervisory approval. Pipe overhead is a vent stack. No rood drains run over head.All other items were in order. No County legal action will result from this inspection.Printed on site.

First walk through with the superintendent. The Following items were discussed:1). Front service area and storage room will be based out with coved base.2). Front service and storage room will have smooth and and easily cleanable surfaces for floor and walls. Same for restroom.3). Water heater will be on a stand in the corner.4). The storage area will have a hard lid ceiling. Paint with a washable ceiling.5). Install a light in the storage room in the ceiling or surface mount..6). All lights in the service area will need to be shatter proof, replace glass bulbs with plastic LED.7). FRP will be installed in the storage room and behind the three compartment sink.8). Install a separate water line with faucet at the three compartment sink, just above the mop sink for possible future chemical dispensers.9). Repair or replace the door sweep on the back door.10). Restroom walls will be painted with a washable paint.11). Restroom door will be made self closing.12). A dual check valve will be required upstream of the Espresso machine. Valve must meet ASSE 1022.13). Recirculation pump may be omitted if wanted but it is advised.14). All vent pipes will be painted black.16). Sneeze guard is not required if you keep all food and food handling along the back wall, you will have to install it when work migrates towards the front. 60" total height with pony wall is minimum.No County legal action will result from this inspection.Printed on site.
