Observed raw fish removed from reduced oxygen packaging in other approved packaging placed in buckets of water with water at 60*F located in prep sink next to three-compartment sink. All foods being thawed from frozen must be thawed in an approved manner. Foods must be thawed under refrigeration, under cold running water, or as part of the cooking process. Person-in-charge (PIC) moved fish to walk-in at time of inspection. Corrective Action: Corrected at time of inspection.

Observed red chlorine sanitizer bucket for wiping cloth; solution reading below 50ppm. Chlorine sanitizer concentration shall be between 50-100ppm. All wet wiping cloths must be stored in sanitizer buckets with approved sanitizer concentrations. Wiping cloths were placed in sanitizer buckets with approved sanitizer concentrations at time of inspection. Ensure sanitizer is mixed well and concentration is tested before use. Corrective Action: Corrected at time of inspection.

Inspection report was emailed toThis establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection conducted with owners.At the time of this inspection, this project was about 60% complete. Inspection report was emailed to Yesica.Prior to final:1. Provide NSF approved hand wash sink with hot and cold water in the area of the relocated ice machine.2. Provide a smooth, cleanable, wipeable ceiling in back areas of kitchen.3. Provide cove base in all areas of bar.4. Provide cove base in restrooms.5. Toilet seats need to be the sanitary break front kind.6. Flash right side of of double walk-in.7. Provide metal cove base along all exposes sides of both walk-ins.8. Seal gaps around mop sink.9. Provide an ASSE 1013 reduced pressure zone assembly upstream of soda system. Have RP tested by a licensed backflow specialist.10. Provide ASSE 1022 dual check valve fore any water fed drink machines (tea, coffee, etc).11. Provide rigid drain lines from all equipment in the bar area. Route to floor sinks with 1 inch air gap above finished floor.12. Seal all fixed equipment at wall junctures.13. Once ceiling installed, flash any opening above walk-ins or wall that are less 24inches high. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company.
