Observed no approved sanitizer in establishment at time of inspection. Per person-in-charge (PIC), they ran out of sanitizer tabs and have not had time to purchase more. Observed ladles hanging on ice well. PIC states they were washed today with soap and water. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Food-contact surfaces must be properly sanitized after washing and rinsing. Employee purchased bottle of bleach at time of inspection. PIC set up three-compartment sanitizer sink with an approved chlorine concentration with inspector at time of inspection. Ladles were properly sanitized at time of inspection. PIC also purchased chlorine test strips at time of inspection. Corrective Action: Corrected at time of inspection.

Observed water temperature at three-compartment sink at 117*F maximum after adjusting water heater. Hot water of: 100°F is required at all handwash sinks; 110°F at all other sinks; 120°F at mechanical warewashing units sanitizing with chemicals; and 140°F at mechanical warewashing units sanitizing with high temperature. Provide hot and cold water under pressure at all sinks prior to re-inspection. Corrective Action: Correct within 24 hours.

Final construction inspection for a Class 2 10+ E&D permit was conducted with owner Luis S. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection for a Class 2 10+ E&D permit was conducted with owner Luis S. by appointment. The establishment was closed and not operating at time of inspection. The permit is not approved. Please address the following items prior to the final construction inspection: 1) Provide 1" minimum air gaps for all indirect drains going to the floor sink. The air gap must be above the finish floor. 2) Elevate the ice machine at least 12-18" inches to provide proper gravity slope to the floor sink. Transition the flex drain to rigid piping, such as PVC. 3) Re-install the FRP behind the ice well and three compartment sink so that the FPR is installed at the top of the cove base. 4) Paint the water heater stand with a semi-gloss minimum paint and then provide rubber cove base around the stand. 5) Caulk the gaps at the bar countertop/wall junctures. 6) Elevate the electronic shelving 6" minimum from the floor or caulk around the base of the unit. 7) Caulk-in all plumbing penetrations and escutcheon plates throughout the facility. 8) Provide self-closing hinges to the restroom and back exterior door. 9) Provide weather stripping to the front exterior door. The Kavafied washing machine unit is not approved for commercial use. Please provide an NSF, commercial approved equipment piece for use. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
