Final Construction inspection conducted with contractor. 3-compartment sink hub drain would overflow when sink compartment emptied. Per contractor, a plumber cleared the drain line and larger hub drain was installed. However, hub drain still overflowed when emptied. There were no nearby floor sinks. Plumber routed and sloped an indirect drain line from the 3-compartment sink to the mop sink with a one inch air gap. Construction completed and approved. Inspection report was emailed to Shane Miller and Rachel.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Arrived to conduct final construction inspection and met with contractor. At this time project is 95% complete. Verified ambient temperature of refrigeration units is 41 degrees F. Verified hot water at handwashing sinks is at least 100 degrees F and at least 110 degrees F at all other sinks.Inspection report was emailed to Shane Miller.Ensure the following is corrected prior final construction inspection:1. Ensure hot water temperature at handwashing sinks maintains at least 100 degrees F, and at least 110 degrees F at all other sinks.2. Ensure refrigeration units are energized and maintain an ambient temperature no more than 41 degrees F.3. Repair slow draining hub drain under 3-compartment sink. Ensure drain does not overflow when sink is emptied.4. Replace clean-out cover below 3-compartment sink.5. Provide a floor cover that is smooth and easily cleanable around grease trap access on floor near 3-compartment sink. Ensure seal cover edges are smooth. Seal around cover.6. Reverse hot and cold water at mop sink faucet.7. Seal bottom of coved base to floor around mop sink, front of the house point of sale area, and restroom.8. Provide indirect drain for espresso machine with a one inch air gap.9. Seal escutcheon plates to walls below handwashing sinks.10. Seal gaps around pipe penetrations in walls.11. Provide detailed cleaning of front/back of the house areas.12. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. Equipment inspection conducted with Site Superintendent. At the time of this inspection, this project was about 85% complete. Establishment has an 80-gallon capacity water heater. Establishment plans to open at the beginning of April 2024.Inspection report was emailed to Shane Miller.Please ensure the following prior to final inspection:1. Ensure hot water temperature at handwashing sinks is at least 100 degrees F, and at least 110 degrees F at 3-compartment sin and mop sink.2. Ensure refrigeration units are energized and holding an ambient temperature no more than 41 degrees F.3. Observed open ceiling in back kitchen dishwashing and prep area. Provide an enclosed ceiling above entire dishwashing area, prep area, hot water heater, and mop sink, which is light in color, smooth, and easily cleanable.4. Install 3/8 inch radius coved base in front of mop sink, perimeter of floor/wall juncture in restroom, and in front beverage prep area at floor/shelving juncture at display and pint sale area.5. Install hub drain with a one air gap at 3-compartment sink.6. Provide rigid indirect drain line from ice machine to mop sink with a one inch air gap.7. Install air curtain with door activated micro switch at back door and drive-thru window.8. Install splash guard to left of beverage prep area dump sink.9. Install FRP or tile on wall to right of dump sink and above/below front prep table.10. Install FRP or tile on wall around handwashing sink in front beverage prep area.11. Paint bare wood under front counter with several coats of semi-gloss or high gloss paint. Ensure surface is smooth and easily cleanable.12. Install ASSE 1022 dual check backflow preventers between water source and espresso and coffee brewers.13. Install/adjust self closing mechanism on restroom door.14. Replace acoustic ceiling tiles in restroom, or paint ceiling tiles with several coats of semi-gloss or high gloss paint. Ensure ceiling surface is light in color, smooth and easily cleanable.15. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. Please continue with construction according to plans, stipulation letter, and inspections.
