The variance application for the alternative sanitization schedule for the F'real machine is approved.1. The facility performs the specified operations in accordance with the approved plans pursuant to §8-103.12(A).2. Alternate cleaning and sanitization schedule is limited to the FRLB4 blender. 3. Logs must be maintained that verify that the F’REAL blender undergoes automatic cleaning and sanitization using the submitted and approved process once every 24hrs and a manually cleaning and sanitizing using the submitted and approved weekly cleaning procedures once every 7 days. 4. The device will undergo descaling to remove hard water deposit buildup inside the blender as needed to ensure device is clean to sight and touch. 5. High temperature sanitization will be verified weekly by heat sensitive indicators/strips, which will be maintained with the weekly logs. 6. These logs must be retained on premises for at least 180 days.7. The facility provides records upon request to the Maricopa County Environmental Services Department (“Department”) or its representatives pursuant to §8-103.12(B).8. A Reduced Pressure Principle Zone (RPZ) backflow prevention assembly is required to be installed. Certify the backflow preventer and provide copy of the backflow preventer test certification.

Final construction inspection.Construction is complete and approved.***The F'real machine must be cleaned and sanitized every four hours until the variance for alterative cleaning schedule is approved. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.
