Equipment Inspection - E&D Class 2This inspection was conducted with the on site manager.A PDF copy of this inspection report was emailed to Howard M. and Theresa B. at the time of this inspection.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, via on-line at ESD.MARICOPA.GOV, US Mail, or phone at 602-506-6824 Option 2. Payment can always be made in person at any regional office. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.No County legal action will result from this inspection.

Per Chapter 1, Regulation 4(a) “No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code.” County legal action may result due to violations observed during this inspection. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.Complete the following prior to final permit approval;1. A new RPZ backflow prevention device has been installed for the water feeding the Arjencia Water machine on the west side of the building. However, this is not the RO system for this facility that is located on the east side of the building. The RPZ backflow prevention device must be installed between the copper piping and the RO system located on the east side of the building. Provide a certified, drinking water safe, RPZ backflow prevention device for the water supply line to the RO system. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass (FC §5-202.14), (FC §4-101.14) A picture of the copper pipe feeding the RO system was provided with this report.

There are currently brass couplings installed on portions of the RO system that need to be replaced with plastic or stainless steel. 2. There is a flexible, waste drain line from the RO system that extends into the P-trap of the floor sink where the brine water drains. Provide a hard pipe drain line such at PVC that drains indirectly to the floor sink with a minimum 1 in. air-gap between the end of the pipe and the flood rim of the floor sink. There is also water leaking into the cabinet where the RO System is installed that must be repaired.3. Provide a certified, stainless steel, RPZ backflow prevention device for the water line feeding the ICEE machines that are equipped with carbonators. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass.4. Neither of the restrooms that open up to the food area have self-closing doors. Provide self-closing doors or continuously running air-curtains for both of the restroom entry ways. 5. The stainless steel, RPZ backflow prevention device for the water supply line to the soda fountain carbonators is not securely attached to the wall and the funnel drain assembly beneath the device has fallen off of the wall allowing water to drip onto the floor. Properly secure and position the backflow device and funnel drain assembly to the wall so that water does not drip onto the floor.6. Provide washable ceiling tiles in the two storage rooms where food and single service items are stored. There are currently acoustical ceiling tiles installed in these rooms.7. Seal the holes on both sides of the wall around the piping and soda lines in the back room and behind the cabinet beneath the soda fountains. The openings should be sealed and finished to be smooth, non-absorbent and easily cleanable. There is also an opening in the wall around the water heater pipes in the back room that must be repaired in a similar way.
