Establishment holds pizza sold by the slice at ambient temperature at display counter using time as a control, however no make times were documented at time of inspection. Employee said the pizzas had all been cooked about 2 hours prior and labeled them with times at time. Please ensure all time/temperature controlled for safety food items which are held using time as a control have the make time documented to ensure a timely discard. Corrective Action: Corrected at time of inspection. Second consecutive violation.

Created written 4-hour time as control written procedure with the person in charge at time of inspection. According to person in charge garlic and oil mixture will be cold held at 41°F or less.

After discussion with the person in charge it was determined the establishment utilizes 4-hour time as a public health control for pizza slices and garlic + oil mixture.

Observed two unlabeled spray bottles filled with clear liquid at time of inspection. One spray bottle was near the handwashing sink. The other spray bottle was near the chemical storage area. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. According to the person in charge the spray bottle near the handwashing sink was filled with sanitizer and the contents of the spray bottle near the chemical storage area were unknown. The person in charge labeled the spray bottle near the handwashing sink as sanitizer and discarded the other spray bottle. Corrective Action: Corrected at time of inspection.

All TCS foods being cooled in deep containers were transferred to shallow containers/ multiple small batches to allow for rapid cooling.
