MCEHC Chap. 8, Sec. 3, Reg. 5c - Mobile food establishments shall provide a commissary agreement to the Department upon request, at permit issuance or renewal, and as a part of the documents submitted during plan review for a new permit. Ice cream vendors that only sell or dispense individually packaged, commercially processed items are exempt from providing a commissary agreement.

Or contact the office if you no longer own the unit.Failure to contact me may result in permit closure.

Operator was unavailable at time of the inspection. Contacted the operator via the phone number provided and left a voice message to call about their location and hours of operation. An email was also sent out to the operator. Please contact us at 602-506-6824 to schedule an inspection or to close out the permit.

Received email from the operator to indicate that the mobile food establishment is not operating and has not been operating. The mobile food establishment will return to operate in Jan 2024. Months have been reduce to reflect months of operation.
