The commissary agreement on file does not allow for fresh water supply and waste water disposal. Per person in charge the fresh water supply is obtained from a personal home and waste water is disposed of at an RV park. Mobile food establishments shall operate from an approved commissary or other food service establishment as required by this Department. Provide a commissary agreement for fresh water supply and waste water disposal at once. Corrective Action: Correct at once.
