Observed an employee's Jimmy John's sandwich in walk in cooler above facility retail food items. Observed another employee's omelet on counter by checkout area. Observed the hand sink in the bathroom without accessible paper towels. The paper towel dispenser was filled but not dispensing. Informed PIC that hand drying provisions are necessary at all hand sinks. PIC provided hand drying provisions at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided hand drying provision for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Observed an accumulation of food debris and residue in the bottom of the microwave. Informed PIC that equipment needs to be cleaned at a frequency which prevents the accumulation of food debris and residue. Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

Observed white build-up in all the soda machine ice dispensers. Observed dark brown and pinkish build up on the inside of one of the dispensers. Observed black organic matter growing of the metal components of the ice dispenser. Informed PIC that the ice dispenser pieces need to be cleaned and remain clean from sight to touch. Discussed with PIC that ice dispensers are food contact surfaces because the ice goes into beverages. All food-contact surfaces must be cleaned to sight and touch. Observed the flooring throughout the entire establishment with heavy scratching, pieces missing, and dark gray discoloration. Informed PIC that the floor should be smooth and easily cleanable. Explained to PIC that the discoloration even after cleaning is evidence that the floor is not smooth and easily cleanable anymore. Discussed the importance of keeping the establishment clean and free from the accumulation of dirt, debris, and organic matter. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Physical facility must be maintained in good repair at all times. Corrective Action: Repair physical facility prior to next routine inspection.
