Repeat Violation American cheese, sliced tomatoes, and shredded lettuce in cold holding unit at grill with an internal temperature ranging 46-51*F. Ice bath holding shredded cheese and cream-based sauces holding at an internal temperature of 48*F. Foods were not fully submerged in ice. Person in charge could not determine time food was entered into units. All cold temperature controlled foods should be held at 41*F or below. Person in charge discarded all food and replaced with product from the walk-in at 41*F. 6-hour discard stickers were placed on product. Recommended replacing unit, or considering time as a control. FOURTH CONSECUTIVE VIOLATION PRIORITY VIOLATION-3-501.16(A)(2) and (B) - Priority: Time/Temperature Control for Safety Food, Cold Holding.
