Observed in bathroom and the mopsink hallway by the back door to have a large square of drywall at the junction of the ceiling and wall that is covered with absorbent drywall mud. Also observed the ceiling paint flaking off exposing drywall and cove base is either falling off or missing along the walls. Discussed with Person in Charge (PIC) all floors, walls, and ceilings must be maintained and repaired such that it is a nonabsorbent, smooth and easily cleanable surfaces.Junctions of wall to floor shall be sealed to prevent entrance of pests. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 3rd Consecutive Violation

Also observed a blue and white buildup rendering the faucet to not be a smooth and cleanable surface. Discussed with Person in Charge (PIC) all equipment must be maintained in good repair and shall be a smooth cleanable surface at all times.
