Observed batter prep cooler closest to warewashing area with door slightly ajar and hanging on its hinges. Observed a gap between the door gaskets. All TCS food in equipment was holding at 41*F. All equipment must be maintained in good repair at all times. Discussed with person-in-charge (PIC) frequent monitoring of internal temperature of food items. PIC states they are replacing the unit in the next week or two. Corrective Action: Correct prior to next routine inspection.

At the time of inspection, containers of blue cheese and a pan of cut lettuce at 46-50*F were cold holding in the refrigerated draws at the cook line. Per manager the items were placed in the draws from the walk in greater than 2 hours earlier. The manager voluntarily discarded the items. Per conversation with the manager, it was determined that the items likely sat out without refrigeration after being removed from the walk-in refrigerator prior to being placed in the refrigerated draws. Discussed with manager ensuring that employees are trained to finish tasks that to ensure TCS foods maintain proper temperature. Corrective Action: Corrected at time of inspection.

Inspector's notes
During the inspection pans of cooked onions and cooked onions with jalapenos in covered pans at 50 to 55*F were observed cooling in the walk-in refrigerator. Per manager the items were cooling for approximately 2 hours. The manager placed the uncovered the items so that proper cooling procedures are followed. Through discussion with manager and reviewing log, temperatures were taken, however they were likely not taken properly to obtain a proper reading, and the items were placed from a sheet pan to the covered containers to soon. Reviewed how to properly take temperatures to verify proper cooling has taken place. Corrective Action: Corrected at time of inspection.

The person in charge indicated they had been placed in the unit 1 hour before inspection. It was determined that the steam table was not on at time of inspection due to electrical issue and the steam table at an internal water temperature of 115*F. The person in charge had these foods reheated to 165*F and reset the steam table so it was on; person in charge voluntarily discarded of the marinara sauce due to repeated reheating affecting quality of sauce. After discussion with the person in charge, it has been determined that the root cause of the violation was a verification issue which resulted in the violation. Person in charge moved the raw beef hamburger patties to a different shelf where they store raw hamburger patties. Person in charge indicated they would coach staff not to store raw animal proteins above any ready to eat foods. After discussion with the person in charge, it has been determined that the root cause of the violation was training staff, which resulted in the violation.
