Inspector's notes
Observed heavy ice accumulation on fire sprinkler head of walk-in freezer. Discussed with person in charge the ice buildup may interfere with proper activation and discharge of sprinkler. Person in charge stated they have a work order in place for the condensation issue. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed unsealed wall penetration around the mop sink drain line. Discussed with person in charge the importance of maintaining all floors, walls, and ceilings repaired such that it is smooth and easily cleanable. Person in charge stated they will put a work order in for it. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Also observed bent metal cove base plate next to the freezer door leaving a gap between wall and the cove base. Discussed ensuring a smooth easily cleanable surface with no gaps with manager. Manager will submit a work order. Corrective Action: Correct prior to next routine inspection.
