At the in-use three compartment sink, no quaternary ammonium was observed in the third vat filled with food pans and various wares. PIC immediately scheduled repair of the sanitizer dispenser. Staff to use the other chemical line dispensing sink and surface sanitizer until the quaternary ammonium dispenser is repaired. All wares were returned to the sink for proper sanitization. Corrective Action: Corrected at time of inspection.

In the hot grab and go case being monitored by using 4hr time as a control, observed two packages of chicken pieces exceeding their 12:30 and 12:36 discard times by over an hour. After discussion with the person in charge, it has been determined that the root cause of the violation was a verification breakdown which resulted in the violation.PIC voluntarily discarded the chicken at time of inspection. Corrective Action: Corrected at time of inspection.
