Observed employee handling raw hamburger patties to cook with gloved hands, patties were placed on grill and then gloves were changed. Employee then began preparing ready to eat salads with new gloved hands, without washing hands first. Discussed need to wash hands after handling raw animal product, and that changing gloves does not substitute proper handwashing before handling ready to eat foods. Please ensure hands are washed after becoming contaminated and before handling ready to eat food items (glove changing cannot substitute proper handwashing). Final construction inspection for a Class 4 10+ E&D permit was conducted with Andrew F. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection for a Class 4 10+ E&D permit was conducted with Andy F. by appointment. The project is approximately 90% complete. The facility is approved to stock and train. Please ensure the following items are addressed prior to the final construction inspection: 1) Provide ASSE 1013 reduced pressure backflow preventers upstream of the bar glass rinsers. Certify and provide test reports. 2) Transition the flex drains at the bar to rigid piping and provide 1" minimum air gaps above the floor sinks. 3) Provide an ASSE 1013 reduced pressure backflow preventer for the future misting system installation. 4) Provide higher splash guards to the handwash sinks. The splash guards must match the highest point of the faucets. 5) Ensure all manual switches are disabled at the air curtains. 6) Provide gaskets or weather stripping to the self-closing Eliason doors at the kitchen entrances. 7) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system, all refrigerators are on and measuring at 41' F or below, and CofO has been provided. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
