Observed chlorine mechanical ware washing unit run several cycles with dirty dishes and produce a strength of 0ppm. When sanitizing with chlorine, solution should produce a strength of 50-100ppm for proper sanitation. Person in charge placed dishes in 3 compartment sink for proper washing, rinsing and sanitizing. PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Predefined Comment: Food-contact surfaces sanitized using chlorine sanitizer must be sanitized with a contact time of least 10 seconds. Construction completed and approved.Inspection report emailed to Ammie B.Please address the following prior to routine inspection:1. Provide an ASSE 1022 upstream of all non-carbonated beverage systems as discussed (drive through area).2. Provide rigid drain line for the mechanical warewash (maintain air gap with floor sink).3. Continue to seal all holes, voids, gaps, cracks, and penetrations (electrical and plumbing - room for walk-ins).4. Adjust the self-closing drive through windows so they don’t stay open when released.5. Label the water lines and backflow at the mop sink as discussed.6. Seal hood vents using heat resistant silicone.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Plumbing inspection conducted with site super intendent. At the time of this inspection, this project was about 65%complete.Inspection report emailed to Jay J.Please address the following prior to next inspection:1. Provide a total of 75-gallon capacity water heater(s) to meet the minimum hot water demands of the facilities’ fixtures. 2. Ensure all water connected devices are protected by backflow or air gap (ASSE 1013 RPZ for steam combi oven at cook line, RO system in the back, upstream of soda carbonator).3. Certify all testable backflow preventers and provide copies of the backflow preventer test certifications. 4. Provide an ASSE 1022 upstream of all non-carbonated beverage systems as discussed.5. Ensure hot water is provided to all sink fixtures within 45-seconds, and measures between 100°F-110°F at the hand sinks.6. Provide rigid drain line for the relief drain line of the water filter.7. Provide self-closers for all restrooms.8. Seal all holes, voids, gaps, cracks, and penetrations (electrical and plumbing).9. Seal all backsplash and sides of all sink fixtures.10. Ensure all chemical dispensers have true air gap 1055A.11. Provide air curtain for delivery entrance and ensure it is activated by door micro-switch.12. Ensure drive through windows are self-closing or protected by an air curtain (micro switched).13. Provide escutcheon plates and cleanout covers where missing.14. Ensure prep sink has faucet installed (currently only had pre-rinse sprayer in box).15. Ensure cove base with 3/8-inch radius is provided for hallway to restroom area. 16. Layout for mechanical warewash machine is in reverse order to the 3-compartment sink (wash, rinse, sanitize). Ensure chemicals are distributed in reverse order to match the mechanical warewash placement, or move mechanical warewash to the right-side of the 3-compartment sink.
