Observed par-cooked chicken parmesan stored above RTE twice-baked potatoes. In the reach in cooler, observed an opened package of raw bacon stored above RTE greens and milk. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. OC re-inspection for a Class 5 Food Catering permit was conducted with owner Edwin R. by appointment. The establishment is still approved to operate. Please address the following items prior to the routine inspection: 1) Trim the foam sealant around the plumbing penetrations and at the splash guard flush and provide a smooth layer of silicone. 2) Caulk around the base of the floor sink box. The owner will potentially be applying for an Eating and Drinking permit to sell grab and go meals directly to customers from the facility location. Discussed the requirement for an additional handwash sink at the north end of the cook line area. Discussed outdoor cooking requirements at time of inspection. All equipment currently being used to cook proteins, fats, oils, and grease is conducted in the electric convection ovens. Any additional equipment to cook these items outside of the electric convection ovens will require the installation of a Type I hood. MCESD inspection report was delivered via email. No County legal action will result from this inspection.

OC inspection for a Class 5 Food Catering permit was conducted with owner Edwin R. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Construction inspection was conducted to assess the facility and status of the permit. Observed the owner present at time of inspection and operating the space at time of inspection. The operator indicated that they had been recently utilizing the space whenever their church kitchens were not available for use. The operator indicated that the space was ready for an inspection and wouldn't provide access at this time. Red sign posted at the facility at time of inspection. The operator was advised that they are not approved to operate the space at this time until approval. An OC inspection has been scheduled for Wednesday December 6th, 2023. MCESD inspection report was delivered via email. No County legal action will result from this inspection.
