Finish installing the funnel drain assembly for the RPZ backflow prevention device for the mister system and finish repairing the floor around the floor sink where this funnel drain assembly will drain.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. There is a brass manifold and a brass coupling downstream from the backflow prevention device for the soda fountain carbonators. All piping and fittings on the downstream side of this device must plastic or stainless steel, no copper or brass. Replace these items with plastic or stainless steel components.2. The cold water valve on the three compartment sink faucet is broken. Repair the three compartment sink faucet so that the cold and hot water valves are fully functional3. The drain line for the soda fountain ice machine in the drive through is not connected. Properly drain this ice machine to the floor sink without connecting to any other drain pipes. Provide a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink.4. Provide a hard piped funnel drain assembly for the RPZ backflow prevention device for the mister system that empties into the nearby floor drain with a minimum 1 in. air-gap.

Ensure that the ice machines for the soda fountains have their own separate drain pipes that drain to floor sinks with minimum 1 in. air-gaps. Do not connect the ice machine drain pipes to any waste drain pipes.2. Do not use the mop sink faucet to feed chemical dispensers. Y- Valves and downstream shut-off devices, including chemical dispensers, cannot be connected to mop sink faucets.3. Provide a certified, RPZ backflow prevention device for all water lines that feed soda fountain carbonators. All piping an fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass.4. Provide a separate, ASSE 1022 backflow device for each coffee and tea machine connected to a water supply line.5. Provide a self-closer or microswitch activated air-curtain for the drive though window.R.S. #1317 was present for this inspection.
