Verified hand sink set-up, cargo trailer, and Cambros/warmers. Permit approvedThis establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. 1) Provide an independent drain line for the soda equipment to the floor sink. Drain line must not be tied into the hand sink drain lines. 2) Provide a funnel drain below the relief valve of the RPZ backflow device. Route drain line to a floor sink. Ensure drain line is rigid. 3) Provide an ASSE 1022 dual check valve upstream of the coffee makers and minute maid/fuze equipment. Each equipment will need one. Total of 4. 4) Test and certify all backflow device and provide the test report. 5) Repair the leak underneath the 3-comaprtment sink drain line. 6) Provide the bell-shaped cover to the AVB backflow device near the garbage disposal. 7) Repair the cold-water handle on the pre-rinse sprayer next to the dish washer. 8) Repair the hot water handle on the hand sink at the cook line. Provide a metal hose clamp. 2) A strong sewer odor was noticed near the dish washing machine near the clean-out. Have a plumber check for any plumbing issues. 3) Re-connect the micro switch to the back door of the restaurant. 4) Repair any holes within the walls, add missing rubber vinyl cove base/tile cove base throughout. 5) Provide brighter light bulbs (LED) and replace all missing/burnt out lights.
