OC inspection conducted with EHS Manager and OC supervisor.Permit approved.Inspection report emailed Brendan and Alan.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained 1. Provide a dedicated waterline for the mop sink chemical dispenser and repair the AVB of the mop sink faucet. 2. Have all RPZ backflows recertified and provide the test reports. 3. Seal the backsplash and sides of all sink fixtures (including restrooms). 4. Seal the juncture between toilet sink basin and floor in all restrooms. 5. Please adjust the mixing valves for hand sinks so they measure between 100°F -110°F (Can warmer, Batch/Blend, Canning line, and outer chemical rooms). 6. Adjust the water flow of the main faucet on the 3-compartment sink (hot side). 7. Repair the leak on the CIP SKID as discussed. 8. Repair the leak on Permeate tank 1500 as discussed. 9. Provide rigid drain lines for all PEX lines in the Water room area (like pumps 1710 and 1720). 10. Repair the leak in the Wastewater room for the Strainer Assembly Model 50. 11.
