Final Construction inspection conducted with owner Sarah A. Construction completed and approved. ***A remodel permit will be created for the addition of the walk-in freezer. ***Establishment is not open to the public. All pick-ups are during a scheduled appointment time.***Establishment is approved to conduct cooking class demonstrations during non-business hours under this permit. Permit is approved to sell bakery goods and meal-prep food items that are sold directly to the consumer (no wholesale). Inspection report was emailed to Sarah A.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. Equipment inspection conducted with owner Sarah A. At the time of this inspection, this project was about 80% complete. ***E&D permit may need to be changed to a food processor permit or an additional permit may be required. ***Discussed requirements for outdoor cooking and a minor remodel application with owner.Inspection report was emailed to Sarah A.Correct/address the following items prior to final inspection:1) Provide a floor sink for the prep sink, 3-compartment sink and condensate drain line. Ensure all waste still goes through the grease trap. Provide 3 separate rigid drain lines that drain into the floor sink with a 1-inch air gap. 2) Change out the ceiling tiles within the restroom and above the water heater and mop sink. Ceiling tiles will need to be washable vinyl.3) Extend FRP 12-inches above the mop sink faucet on both sides.4) Extend the cove base along the wall of the water heater and around the mop sink basin.5) Provide a 3/8” metal cove base to the bottom of the hood wall, and outside and inside of both walk-ins. 6) Add high temperature silver colored silicone to all gaps within the hood. Remove old silicone first.7) Seal in all pipe penetrations and add CAM buttons to the holes within the walk-ins.8) Provide an ambient air temperature of 41’F or be low for all cold holding equipment.9) Provide 100-110’F of hot water to all hand sinks and at least 110’F of hot water to all other sink fixtures.10) Provide the sign off for the hood and the C of O from the city of Mesa.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No county legal action will result from the inspection.
