Observed shellstock (oyster) tags without the "out" date. A box of oysters in the walk-in refrigerator was found without a tag. Also observed oysters in two separate refrigerators, which per PIC, were from the same batch. The tag was already filed away with only a date indicating when the box was opened. Instructed PIC to keep the tag with the oysters and to mark an "out" date when last oyster is sold, then filed in chronological order. A batch of oysters needs to stay together, can not be separated. Provide new seal for the walk-in freezer. The side has gap that allows for air to enter the unit and ice to build up inside the door.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. At the time of this inspection, this project was about 70% complete.Inspection report emailed to Nasser A.Please address the following prior to next inspection:1. Provide a smooth easy to clean ceiling above the meat market area and ice machine (L shaped as discussed).2. Provide an NSF approved hand sink with splashguards adjacent to the prep and 3-compartment sink.3. Repair the leak from the meat display case cooler.4. Provide air gap between the floor sink and display case drain.5. Provide lighting in the mop sink area as discussed.6. Seal all electrical and plumbing penetrations (mop area and restroom).7. Provide 1-inch air gap for the water heater pan drain and mop sink.8. Seal the toilet basin and floor juncture.9. Provide cove base for the dry storage area and corner of store front.10. Provide several coats of semi-gloss paint for the dry storage area.11. Provide a door sweep for the rear exit (no light shining through).12. Ensure to check with local city jurisdiction for grease trap and fire sprinkler clearance for ceiling installation over meat market. Please continue with construction according to plans, stipulation letter, and inspections.
