Inspection report was emailed to Danny.Reinspection required, schedule for May 28 @ 2pm:1. Provide hot or tempered water at food prep sink as discussed. This can be through a separate faucet, a mixed faucet, a mixing valve set to 110°F etc.2. All sinks will be caulked at splash guards.3. Finish installing back door transition.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.A certified food manager is required at time of approval. Pre-final walk through. The following items were discussed:1). Please remove all the copper from downstream of the backflow valve and the soda system and icee machine tie directly into the valve without coper or brass fittings.2). Floor sinks and drains that will not get a lot of water will be outfitted with trap seals. 3). Steamers will be receive dual check valves that meet plumbing standard ASSE 1022.4). Back door door frame will be finished out.5). Please finish out the edges around the safe and edge of the counter.6). Long power cord for the icee machine needs to be shortened up and tucked away.7). The drain line of the Icee machine may not run horizontally as a flex line. Transition at the closest possible point to rigid and lead t the floor sink, Maintain gravity flow and air gap.8). Counters and about all material junctures will be caulked in.9). Finish the wall penetration of the icee / soda bundle in the drive through area.10). Hood and grease evacuation tank edges will be caulked in.11). All sinks will be caulked at the top wall juncture and splash guards.12). No splash guards needed at the back room hand wash sink.13). Grease tanks will be caulked at the floor junctures.14). Backflow valve for soda system will need to be tested and certified.15). Pressure relief from water heaters will be spaced from the wall.At time of final make sure you have hot and tempered water at all sinks, refrigeration equipment is on and holding at proper temperatures.Note: Hose reel does not need a backflow valve. There is no stopper in the reel, Counter top pitcher rinser does not need a backflow valve as the inlet is raised an inch above the catch tray.Store, after city approval, okay to train in the store. Second walk through of the White Castle. The following items were observed:1). Front counter will be flashed at the side walls where open.2). Restroom counters will be caulked in.3). Coved base will be installed along the toe kick of the front counter on the kitchen side.4). See if the front drove through counter cut outs at the outlets can be painted or sealed.5). Please flash off the side of the safe at the counter junctures. 6). Escutcheon plates will be installed where missing.7). Drain lines at the ,op sink will be cut back and air gap provided.8). After tile is finished the mop sink will be caulked in at the wall junctures, please make sure to provide a flush with the sink or beveled away from the wall bead of caulk.9). Wood in the walk-in floor will need to be sealed, gray painted with a semi gloss or better.10). All cam hole plugs will be installed in the walk-in units were missing.11). All penetrations through the walk-in walls will be filled in and finished out on the inside.12). If the spray arm at the three compartment sink does not get lifted by a spring then install a Watts LFN9 or T&S B-09070-FE, or equivalent dual check valve on the assembly.13). In water heater room, all wall penetrations to the exterior will be sealed and finished out.14). All fly fans will be micro switch activated.Backflow comments:15). Reduced Pressure Zone (RP) backflow valve (ASSE 1013) will be installed upstream of the soda system in the mop room.16). The hose reel (HR) will not need a backflow valve as the reel automatically retracts and can not be stopped.17). Backflow for the steamer is as specified in plan.18). Per plan, all non carbonated beverage machines will receive a DC-1 dual check valve that meets plumbing standard ASSE 1022. 19). Per plan or install a dual check valve DCV-1 upstream the blenttec rapid rinse station specified and steamers. 20). Testable backflow valves will need to be tested and certified.21). First walk through with the superintendent. Location is about at top out. Only the filtered water lines which will be CPVC are not in place yet.The following items were discussed and or noted in the plans:1). Reduced Pressure Zone (RP) backflow valve (ASSE 1013) will be installed upstream of the soda system in the mop room.2). The hose reel (HR) will require a backflow valve upstream if the unit does not come with an integrated spill proof pressure vacuum breaker.If the unit does not have an integrated backflow install a double check valve on the water supply. Valve will need to be installed in a testable location between 12 inches and 5 feet from the floor.3). Backflow for the steamer is as specified in plan.4). Per plan, all non carbonated beverage machines will receive a DC-1 dual check valve that meets plumbing standard ASSE 1022. 5). Per plan or install a dual check valve DCV-1 upstream the blenttec rapid rinse station specified.6). Floor sink at the dining room can brought half out as the soda station is leg mounted.7).
