Final construction inspection. Construction complete and approved.Permit issued Eating and Drinking Class II 0-9 Seating.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. Pre-final walk through. Store has hot water, backflow valves are in place and have been tested and the refrigeration units are in and holding at temperature.Superintendent will follow up on:1). Installing the hot water faucets.2). Finishing up the ice machine plumbing, waste and water lines and installation of the ice dispenser.3). Drive trough window will receive microswitch. 4). Backflow valve will be re-mounted under west side rinser.5). Please make sure all chemical dispensers are being installed at the sinks.Store is okay to stock and train after City TCO.No County legal action will result from tis inspection.E-mailed inspection report.

Second walk trough of the establishment with the superintendent. The following items were discussed:1). Schluter will be installed in the restroom.2). Hub drains will be installed under the backflow valves at the mop sink.3). Ice machine will be rigid drains.4). Flooring will be touched up and all seams closed, also at fire riser.5). Not sure if steamer is water connected. If it is please provide a RP upstream.6). Fly fans will be outfitted with micro switches.7). Hans wash sink splash guards will be siliconed to the walls.At time of final, please provide hot water at all sinks, all equipment needs to be in place, refrigeration should be on and holding at correct temperatures.No county legal action will result from this inspection.E-mailed report
