New permit inspection or the Snow Fruit # 667 operating out of the Fry's produce area. Walk through conducted. Please make the following corrections before your re-inspection:1). Re-mount the food prep sink and caulk the top wall juncture.2). For both sanitizer rinse dispensers at the three compartment sink and the food prep sink, provide an air gap in the backflow housing of the eductor.Company will cut fruits and vegetables in different combinations and package the fruit for onsite sales. Sales will be from display cases or bunkers depending on the type of product. The company will replace the fruit and vegetables cut in site by Fry's staff.Operator will make use of one prep table, food prep sink, three compartment sink, hand wash sink, storage in the walk-in coolers.Dry goods will remain packaged and stored above the prep table.Labels will reflect appropriate expiration date, business name and contact information.After expiration, if occurs the company will cull the expired items.Company procedure dictates cold holding temperature verification.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail.
