Final Construction inspection conducted with site superintendent Trent W. Construction completed and approved. Inspection report was emailed to Trent W.***Recommended to owner Mario to add a sneeze guard in front of the dipper well. Do not display any open food or drinks along this side of the counter top. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Conducted scheduled onsite inspection with owner Mario C. and Paulo P. Inspection report emailed to Paulo P.***Establishment’s menu and process does not have any produce/food that will need to be washed or thawed. If process changes in the future a food prep sink may be required. Correct the following items prior to final inspection:1) Provide a solid surface countertop (i.e. quartz, marble, Corian, granite, stainless-steel) in the front service area. Laminate counter tops are not approved. 2) Provide the C of O from the city of Gilbert.3) Provide the backflow test reports.4) Install FRP to the bottom cabinet and below the ice machine. 5) Finish installing cove base throughout the front service area and to the toe kicks of the cabinets. Install cove base to the outside of the mop sink basin.6) Ensure all cold holding units are turned on and have an ambient air temperature of 41’F or below. 7) Provide a bead of silicone to the bottom of the Schluter cove base in the restrooms. 8) Provide escutcheon plates where missing and caulk in all escutcheon plates. Equipment inspection conducted with Paola P. At the time of this inspection, this project was about 85% complete. Inspection report was emailed to Paola P.***Per Paola P., they will not move forward with a dog friendly patio. Discussed requirements to seal pavers, 7-foot distance requirement and signage. Correct the following items prior to final inspection:1) Provide either and RPZ (ASSE 1013) or double check valve (ASSE 1015) for the batch freezer.2) Provide an RPZ (ASSE 1013) backflow device upstream of the misting system. Backflow must be installed no greater than 5 feet from the floor. 3) If espresso machine will be connected to water provide a dual check valve backflow preventer (ASSE 1022).4) Test and certify all testable backflows and provide the test reports.5) Provide the C of O from the city of Gilbert.6) Provide FRP from the finished floor to at least 12-inches past the drying rack behind the 3-compartment sink. 7) Provide FRP around the mop sink basin. FRP must be installed on the left wall and behind the mop sink up to the height of splash (12-inches past the mop sink faucet).8) Provide a semi or high gloss paint throughout the establishment: front service counter, entire back room, restroom ceilings, doors, and mop sink/water heater room. This also includes ceilings. 9) Provide a 3/8” cove base to the wall and floor juncture of the front service area, to toe kicks of cabinets, back room, restrooms and mop sink/water heater room. 10) Provide 100-110’F of hot water to all hand sink, and at least 110’F of hot water to all other sink fixtures.11) All cold holding equipment must be on and have an ambient air temperature of 41’F or below. 12) Adjust the pre rinse sprayer so that it does not go below the flood rim.13) Provide a 90 degree PVS elbow for the drain lines below the MEHEN equipment.14) Provide details on the production of the Gelato and where the Gelato comes from. Provided handout on frozen desert requirements in regards to ice cream bases. 15) Seal around the voids of the mops sink basin. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. The Environmental Services Department will begin emailing permit card(s) and permit invoice(s) to permittees in the coming months.
