This establishment is not participating in the award program at this time and had 0 Priority, 0 Priority Foundation violations on this inspection.***White Frigidaire is only approved for non TCS foods (Pretzels).***Large reach in prep table will only be used for spices and dry goods.Chemical dispenser at the 3-compartment sink will be removed. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. OC Inspection for E&D, Food Processor and Catering permit. Inspection conducted with owner Maria V.**White Frigidaire is only approved for non TCS foods (Pretzels). ***Large reach in prep table will only be used for spices and dry goods. Establishment was not open and operating.Correct the following items prior to reinspection on 12/8/23:1) Provide a reduced pressure backflow device (ASSE 1013) upstream of the RO system. Ensure there is no copper or brass (lines or fittings) downstream of the backflow device and RO system. Provide PEX, plastic or stainless steel. Test and certify all testable backflow devices and provide the test report.2) Provide a dual check valve (ASSE 1022) backflow device upstream of the espresso machine. Ensure the backflow device is stainless steel or plastic. Ensure lines and fittings are PEX, plastic or stainless steel. 3) Provide an internal air gap (ASSE 1055A) for the sanitizer chemical dispenser. Flex gaps (ASSE 1055B) are not approved.4) Paint the ceiling in the restroom a light color. Provide a semi or high gloss paint. 5) Cap the 3-compartment sink drain line from the hand sink. Route an independent drain line for the hand sink to the floor sink.6) Provide an “employee only” sign for the residential refrigerator in the back room.7) Provide a self-closer for the restroom door.8) Readjust the door sweep for the back door. Observed light shining through the bottom of the door. 9) Provide a temporary hand sink set-up for the catering permit. 10) Provide labels for the pretzels. Labels must include the ingredients in descending order, address of establishment where pretzels are made, and any allergens.
