Observed the following food items cold holding at an internal temperature of 54-55 deg F in a walk in unit that was holding >41 deg F Marinara and pizza sauce,hard boiled eggs, deviled eggs, cream cheese, goat and red wine goat cheese. and bacon. These food items had been in unit since the night prior according to the manager and were discarded. Havarti, Gouda, and provolone, salami, and ham had been placed in unit from a delivery a short time prior and was redirected to alternate cold units within the establishment. Embargo was prepared. Final construction inspection for a Class 3 10+ E&D permit was conducted with superintendent Adam D. by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection for a Class 3 10+ E&D permit was conducted with superintendent Adam D. by appointment. The project was approximately 95% complete. Please ensure the following items are addressed by the completion of the project: 1) Remove the copper piping downstream of the soda RP backflow preventer.2) Provide a swing faucet in conjunction with the sprayer faucet at the three compartment sink. All compartments must be able to be filled by a swing faucet. 3) Disable the manual switch on the back exterior door air curtain. 4) Ensure the flex drains at the ice machine and kegerator are transitioned to rigid piping and air gapped above the floor sink. 5) Provide gaskets and weather stripping to the kitchen Eliason door. 6) Provide self-closures to the restroom doors. 7) Caulk the base of the mop sink with silicone. 8) Provide a 3/8 inch minimum cove base to the brick column areas at the bar interior. Utilize a silicone grout. At the final construction inspection, ensure hot water measuring 120' F minimum is provided to the plumbing system and all refrigerators are on and measuring at 41' F or below. Reviewed sink fixture locations, floor sink locations, backflow prevention, and finishes. Please ensure the following items are addressed by the completion of the project: 1) Provide a 3/8 inch minimum metal cove base to the exterior of the walk-in cooler. 2) Provide a clear matte brick sealer to the brick columns within the bar areas. 3) Caulk-in all escutcheon plates and plumbing penetrations throughout the facility with silicone.
