To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law 1) Install an RPZ backflow preventer (ASSE 1013) on water line to RO system. Ensure a drain line is run from backflow to nearest floor sink with a 1” air gap. Have backflow tested after installation and maintain certificate on site for verification. 2) Have RPZ backflow to soda system and to water line to blender/glass rinser tested. 3) Install an ASSE 1022 backflow preventer on water lines to all coffee/tea brewers. 4) Install an ASSE 1022 on water line(s) to steamer(s). 5) Install a hand sink in ware washing area. Hand sink must have hot and cold water and drain to the sewer. 6) Provide a riser (will provide picture) on drain line to 3-compartment sink, to provide an indirect connection (no floor sink available). Recommend installing on left side of sink. Ensure riser terminates 1” below bottom of basin of 3-compartment sink. 7) Ensure all drain lines are hard-piped, sloped to drain, with 1” air gaps above floor sinks (drive-thru soda area, RPZ for blender/glass rinser in beverage area).
