-- Inadequate hand soap available for handwashing for 1 of 1 hand sinks in the kitchen with the hand soap being significantly diluted with water. Handwash sinks must be supplied with hot water of at least 100°F, approved soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. The PIC provided new bottle of soap for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection conducted with Sara, . At the time of this inspection, this project was about 85% complete. Inspection rights and inspection report was emailed to SaraAddress the following items prior to final inspection:1. Provide 50 gallon hot water heater2. Provide hard piping for the ice machine. Flex hose can go into hard pipe that provides 1 inch air gap to the floor sink3. Provide cove base in the restrooms and around the mopsink4. Seal all escutcheon rings, cleanout covers, penetrations. All surfaces are to be easy cleanable, durable, non-absorbent and smooth5. Complete install of the grease trap cover and seal. Area not to be able to retain water6. Seal with semi-gloss the underneath of the restroom counters7. Seal top of the restroom countertops. Provide hard surface that beads water8. Seal mopsink floor. Floor junctures to be flush9. Provide Frp or stainless steel at the mopsink area. Height to be above the faucet providing a backsplash area and the width of the mopsink on both sidesFacility approved for stock and trainInspection scheduled for Jan 10 at 1pmPlease continue with construction according to plans, stipulation letter, and inspections.
