Final Construction inspection conducted with owner Robert L. Construction completed and approved. Inspection report was emailed to Robert L.***Suite next door is only approved for the storage of the mobile unit. No prep shall be done within this suite. City of Mesa has approved this suite as a commissary to dump their waste.Correct the following items prior to the next to routine inspection:1) Provide a hose bibb vacuum breaker to each water spigot near the water heater. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No County legal action will result from this inspection.

Equipment inspection conducted with Site Superintendent Paul W.. At the time of this inspection, this project was about 75% complete. Inspection report was emailed to Paul W.Correct/address the following items prior to final inspection:1) Finish installing the RPZ backflow device upstream of the RO system. Provide a hub drain below the relief valve of the RPZ backflow device and drain to the floor sink. Do no install any copper or brass fittings, lines, or connections downstream of the RPZ backflow device and RO system.2) Test and certify the backflow devices and provide the test report.3) Provide a semi or high gloss paint light in color to the walls in the storage room.4) Provide washable vinyl ceiling tiles above the mop sink.5) Provide a self-closing door in between the two suites and the restrooms.6) Finish installing the sneeze guards and side returns.7) Install a dry rack above the 3-compartemnt sink.8) Provide the C of O from the city of Mesa. 9) Provide a stainless-steel ASSE-1022 dual check valve backflow device upstream of each non-carbonated beverage equipment (i.e espresso machine, drip coffee, etc).10) Ensure all cold holding equipment is 41’F or below.11) Provide 100-110’F of hot water to all hand sinks, and 110’F of hot water to all other sink fixtures.12) Relocate all equipment and storage used for the coffee shop into the permitted area and not in the adjacent suite where the trailer is kept. Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. No county legal action will result from the inspection.

Plastic construction walls were added. Construction has begun and underground work has started. Please continue with construction according to plans, stipulation letter, and inspections.
