Inspector's notes
Observed hot water heater gushing out water from a large leak near the base of the unit near the pan. Water was diverted from the pan to the mop sink by way of a plastic tube. Tube was also leaking near the connection with the pan and was being caught by a mop bucket placed underneath. Improper repair and leakage must be properly repaired by re-inspection on 5/27/2026. Corrective Action: Correct prior to re-inspection.

PRIORITY VIOLATION-5-202.13 - Priority: Backflow Prevention, Air Gap Addl Notes: 1. The new RPZ backflow prevention device installed for the RO system is installed improperly downstream from the RO system, in a vertical position and there is no funnel drain assembly that empties into a floor sink installed beneath it. Properly install this RPZ backflow prevention device upstream from the RO system in a horizontal position and provide a funnel drain assembly that emtpies into a floor sink for it. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass. 2. The waste drain lines from the RO system and for the open face produce cooler are each directly connected to the sewer system through the drain pipe of the back hand sink. Each of thes drain lines must empty indirectly into to floor sink with a hard pipe drain line and a minimum 1 in. air-gap between the ends of the hard pipes and the flood rim of the floor sink they drain into. 3. Provide an ASSE 1015 double check valve backflow preventer upstream of the proofer oven. 4. Provide an ASSE 1022 dual check valve backflow preventer upstream of the tea brewers. 5. Reroute the wash and rinse drain lines from the 3-compartment sink to the floor sink. If floor sink is not connected to the grease trap than provide a riser to the left side of the wash basin. If riser is installed, riser must be below the 3-compartment sink basin. Corrective Action: Correct prior to re-inspection.

This establishment is not participating in the award program at this time and had 1 Priority, 0 Priority Foundation violations on this inspection.Inspection report emailed to Jen R.Correct all items on the report prior to reinspection on March 28th.***Observed a newly designed front service area with new equipment and new restrooms finishes. Establishment remodeled back in 2018 without prior approval from the Department. A Remodel permit will be required for this location.***Provide an NSF/ANSI approved prep sink in the back of house. Prep sink must drain to the nearest floor sink. Drain lines must be indirectly plumbed with an approved minimum 1-inch air gap. Ensure prep sink is within 25-fee of the hand sink. (MCEHC §8.1.8g (1), (a))***New open reach in cooler must only be used for uncut produce and pre-packaged items only. Do not store any cut lettuce or cut tomatoes in this cooler. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Provide a funnel drain below the relief valve of the backflow device and drain to the nearest floor sink. 2) Provide an ASSE 1015 double check valve backflow preventer upstream of the proofer oven. 3) Provide an ASSE 1022 dual check valve backflow preventer upstream of the tea brewers. 4) Reroute the wash and rinse drain lines from the 3-compartment sink to the floor sink. If floor sink is not connected to the grease trap than provide a riser to the left side of the wash basin. If riser is installed, riser must be below the 3-compartment sink basin.
