Observed several dishes stored at the clean dishes rack with water observed still between the nested dishes. The person in charge separated the wet dishes so they could finish air-drying. All equipment and food contact surfaces must be air-dried after properly cleaning and sanitizing. Corrective Action: Corrected at time of inspection.

Observed a handwashing sink across from the fryers that was draining extremely slowly/clogged. Discussed with person in charge to call a plumber to service the handwashing sink and to utilize other nearby handwashing sinks until the sink in question is unclogged. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained 1. Provide an ASSE 1015 DC backflow preventer upstream of Hydra cleaning systems of the ice cream makers. 2. Provide a drain line for the Hydra cleaning systems. Drain lines are not permitted to discharge into buckets or similar temporary containers. All drains must properly discharge into an approved sewer through a floor sink or floor drain. 3. Provide uni-struts for drain lines so that air gap with floor sink may be maintained. 4. Provide an air gap for the soda drain line at the drive through window. 5. Have all backflow preventers certified by backflow specialist and provide the test reports (2 RPZ at BIB, and 2 New DC backflows). Correct By: Correct Prior To Reinspection
