Please ensure and RPZ ASSE 1013 is on water line to unit and tested . Recommend: adding cove base to side walk way and below three compartment sink. Seal side gaps. Repair leaking ice machine. Remove duct tape from ice machine; grey Duct tape is not cleanable surface. Disconnect freezer unit. Adding CO2 sensor monitor in employee storage area.

Cut lettuce and kraft macaroni and cheese-46-49F in the table top area of the make line unit on the left side of the cook line area. Employee stated that both items were recently placed into the unit. PIC relocated the macaroni and cheese to the walk in cooler and voluntarily discarded the lettuce-see embargo form. Keep PHF/TCS foods in cold holding at 41F or below. After discussion with the person in charge, it has been determined that the root cause of the violation was a training staff and a verification breakdown which resulted in the violation. Corrective Action: Corrected at time of inspection.
