Observed chemical dispenser attached directly to mop sink with an atmospheric vacuum breaker. Faucet was observed to be turned on at time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Non-testable anti-siphon vacuum breaker devices are designed to protect potable water supplies from the back siphonage of non-health hazard cross-connections and low flow, non-continuous pressure installations. Establishment is observed to have a designated water supply for the chemical dispenser. PIC turned off water and removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Observed melted dairy butter next to stovetop at 125*F-130*F. Per person-in-charge (PIC), butter was melted approximately 1.5 hours prior. Except during preparation, cooking, cooling or when time is used as a control, all TCS foods must be maintained at 135°F or above. All TCS foods improperly hot held were reheated to 165°F at time of inspection. No documentation was provided at time of inspection stating butter is non-TCS. Corrective Action: Corrected at time of inspection.
