Observed bidet directly connected to toilet water supply without an approved backflow device in employee restroom. High hazard equipment connected to potable water sources require a high hazard backflow device (approved reduced pressure zone assembly backflow device) that meets ASSE 1013 standards. Provide an approved backflow device or remove bidet prior to re-inspection. All testable backflow devices must be certified. Corrective Action: Correct prior to re-inspection on 8/5/2025.

PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law>>>Observed bathroom sink with no discharge piping. Person in charge indicated they cannot use the sink and have been using the 3-compartment sink to wash their hands after using the restroom. The establishment does not currently have any food contact surfaces and their mop sink is currently occupied by boxes/miscellaneous objects. The person in charge indicated the bathroom sink has been down for a week while the operator finds a plumber to fix it. Person in charge was informed they must repair the bathroom sink by re-inspection; person in charge was also informed that they should not use their 3-compartment sink for hand washing; if they want to incorporate food contact surfaces back into their operation they must thoroughly wash, rinse, and sanitize each basin of the 3-compartment sink prior to bringing back food contact surfaces (such as drink nozzles, ice scoops). Inspector suggested removing items from mop sink and using this for hand washing until the bathroom sink can be repaired. RE-INSPECTION 04/01/24. Correct By: Correct Prior To Reinspection
