Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home. Predefined Comment: No violations noted at time of inspection. Discussed with Operator installation of new Exhaust hood system, three compartment sink, and large 2-door freezer unit located at rear kitchen area. New exhaust hood was not in operation at time of inspection. No gas equipment observed to be installed at time of inspection. Per operator, food establishment plans to open a kitchen in August 2025. Operator was unable to provide details if exhaust hood wasapproved by Maricopa County Environmental Services prior to installation. Operator was instructed to apply for a remodel via Permit Center within 5days.
