Observed tomato juice measured at 60F in main bar cooler. PIC reported cooler not holding temperature below 41F and should not have been used for TCS items. PIC reported tomato juice had been in bar cooler since yesterday. All tomato juice improperly cold held was discarded by PIC at time of inspection. Observed michelada mix (with cooked tomatoes and onions) used for drinks stored at the bar area at a temperature of 67 F. Per PIC, item had been out of temperature control for over four hours. Michelada mix was voluntarily embargoed by PIC at the time of inspection. Also observed cut lettuce at a temperature of 48 F located in the tortilla prep area. Per PIC item was prepped and placed in reach-in cooler an hour ago. Informed PIC that items should be placed in the walk-in unit prior to being placed in reach-in coolers because unit doesn't have the capacity to cool items. Informed PIC that except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Corrective Action: Corrected at time of inspection.

Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist. PIC closed the front door at the time of inspection. Observed deep covered pans of guacamole (with tomatoes) measured 46-48'F in walk-in unit. Observed a layer of saran wrap covered between food and container lid, resulting in an extra layer of trapped heat. PIC stated food items were prepped at ambient temperature the day prior and have been cooling in walk-in unit since for longer than 4 hours. All TCS foods improperly cooled were discarded by PIC at time of inspection. TCS foods prepared from cold or ambient room temperature ingredients must be cooled from 70°F to 41°F within 4 hours. Avoid covering cooling items to maintain proper air flow and prevent trapping heat. Provided guidance documents on cooling and proper cooling methods via email. Ensure scooping equipment are equipped with handles to prevent contamination from the hands. All in-use utensils were properly stored at time of inspection.
