Observed a double "Y-valve" shutoff installed on faucet of mop sink, with one end connected to a chemical dispenser, downstream of the atmospheric vacuum breaker on the mop sink. Instructed employees to have y-valve disconnected and reviewed proper plumbing connections. Ensure all plumbing is connected properly and maintained in good repair. Instructed to have Y-valve removed at time of inspections and reviewed proper plumbing connections. Corrective Action: Corrected at time of inspection.
