Observed a buildup of flour on surfaces, shelves, and on the floor in corners. Cleaning should be frequent enough to prevent accumulation. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed vents that have a buildup of dust, and dirt above food production areas. Also observed a few vents, that are open to the attic crawl space with little protection from falling debris. These vent covers also have buildup from dust/dirt. Please have the vents cleaned to reduce physical contamination on food production lines. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Observed raw whole wheat flour in a covered storage bin with 5-10 black specs visible at the back of the containers. Upon further inspection, observed small black larvae resembling weevils and shed skins. Per PIC, bread production has been limited during the warmer months and the wheat has not been is use in customer foods. All food must be in good condition, safe, and unadulterated. All adulterated food was discarded at time of inspection (see embargo). Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed the northeast door opened with a magnet screen door in use that does not provide adequate protection against pest entry. Observed gaps in between magnets and an unprotected space at the base of the door approximately 2-3" high. Advised PIC to obtain a fixed metal security door or similar door. Discussed gaps may allow pest entry and should be filled with door sweep or weather guard prior to the next routine inspection. Corrective Action: Correct prior to next routine inspection. *Second repeat violation* 6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed 30+ hairy larvae of various sizes in flour debris under the rubber floor mat in front of the 3 compartment sink. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.

Core-6-202.15, C: Outer Openings, Protected ------ Observed the back door propped open without the aid of air curtains, a screen door or similar device to help prevent pest entry. Disable the manual switches on the drive-thru air curtain units. PIC stated they were advised by their HVAC professional to open a door to assist the swamp cooler functionality. Advised PIC to keep the door closed until an effective barrier against pest control can be added. The manager closed the back door at the time of inspection.
