During inspection, a QUAT mechanical warewasher was observed actively being used to sanitize equipment, with a QUAT sanitizer level below the manufacturer's instructions/levels. The person in charge verified that the chemical concentration was below the appropriate levels using the facility's test strips and submitted a work order at the time of the inspection for the machine. The facility will rely on alternative means of sanitizing equipment until the appropriate chemical concentration can be established and verified. All equipment was rewashed and sanitized at the time of the inspection. Please ensure warewashing is conducted with sanitizer at the appropriate chemical concentration. Final Construction inspection conducted with site superintendent, Tim, food safety director, Kerri and management, Ahsha. Construction and management team walked site also.Remodel permit is required to finish main dishroom as discussed. SOP for Cleaning hoses has been provided. Per management, hoses are not be left connected while cleaning is not occurring. Floor cleaning hoses are to be disconnected downstream of 1024 after use-otherwise PVB is to be provided. All refrigeration was holding at 41 or below. Backflows tested and certified. Construction completed and approved. Inspection report was emailed to Kerri and Ahsha.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Equipment inspection conducted with Site Superintendent, director of food safety and Design team. At the time of this inspection, this project was about 80% complete. Inspection report was emailed to Danny and Tim.Please continue with construction according to plans, stipulation letter, and inspections. Prior to final:1. Provide all backflows as previously discussed with RS #658. All backflows are to be in place and tested prior to final. Provide test reports.2. Provide cove at the floor wall juncture under the hoods.3. Seal all metal base along all walls at floor juncture (chemical storage, veg/meat storage, cook, packaging etc).Noted gaps throughout.4. Seal all penetrations are ceilings throughout.5. Provide dedicated water line at mop sink as previously discussed.6. Seal all saw cuts and expansion joints in flooring (noted at most door ways).7. Seal saw cuts around new floor drain in water heater room.8. Provide second faucet at 3 compartment sink in larger dish wash room.9. Ensure soap and paper towels are provided at traditional hand washes.10. All cold rooms for food storage must be holding at or below 41°F.11. Hand wash water must be set for 100-110°F.12. Vegetable prep sink must be hard plumbed to floor sink with 1 inch air gap AFF.Please call (602) 526-0917 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed. Please also ensure you have read your Responsibilities to Qualify for a Permit document and are in compliance with all requirements before final inspection.
