Observed the 3 comp sink drain line stored below the flood rim of the floor sink. PIC stated the drain was recently repaired. A 1 inch air gap must be maintained. Must repair all plumbing to a state of good repair prior to next routine inspection. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection.

No self-closing mechanism observed on the restroom door, which was observed opened at the time of inspection. Except where a toilet room is located outside a food establishment and does not open directly into the food establishment such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight fitting and self-closing door. Observed assorted cheese and soft cheese and cheddar cheese cold holding at internal temperature of 45-46 deg F in a walk in unit that was holding >41 deg F. Manager informed the cheese had been in walk in since the night prior and discarded. Embargo form was written. The walk in unit returned to <41 deg F during inspection.
