Per person in charge (PIC) the chicken was reheated in the microwave and placed in the hot holding equipment approximately 20 minutes prior to inspection. Discussed with PIC TCS food items prepared onsite and reheated for hot holding must be rapidly reheated to 165°F within 2 hours prior to hot holding. Observed large container of raw chicken on the ground in the hallway. Ambient temperature of the room was 79*F and the internal temperature of the raw chicken was 32*F. Per PIC the chicken was brought over from the other location and placed there 1 hour prior to inspection. Discussed with PIC Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Also discussed with PIC foods shall not be stored on the ground. PIC placed on a low shelf in walk in at time of inspection. Corrective Action: Corrected at time of inspection.

Observed several single use large soy sauce containers holding dry rice, teriyaki sauce, orange sauce. Per PIC the sauces arrive from the other store location in the large soy sauce containers that are washed and reused at that location. An empty soy sauce container stored upright by the ice machine is used as the ice bucket. A stack of used and rewashed soy sauce containers are stored in the hallway on the ground by the back door for pick up by the other store location. Per PIC the containers will be picked up and taken to the other store where they will get washed, reused and be used for the next batch of sauces to be brought back to this location. Discussed with PIC these single use containers are not designed to be washed and reused for sauces, dry foods or even as an ice bucket. Also discussed ice buckets shall be stored upside down between uses. Discussed to replace the single use containers with food safe reusable containers. >>>Observed in kitchen prep table top TCS items that had an internal temperature of 54*F to 56*F. The ambient air temperature of the unit top was at 57*F. No food was being stored in the bottom at time of inspection. Per Person in Charge (PIC) the TCS items had been placed in the unit more than 4 hours prior to inspection. All TCS foods improperly cold held were embargoed and discarded by PIC at time of inspection. The TCS items were cooked carrot and peas, canned pineapple, raw egg, soft tofu and cooked broccoli. PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law ------ Observed a star-shaped crack in the basin of the hand sink in the back kitchen so approximately half of gray water from handwashing is allowed to freely run onto the floor or into a bucket stored beneath the sink. Discussed that plumbing must be maintained in good repair and according to law and that all wastewater must drain into an approved sewer system. The establishment has another hand sink near the ice machine in the front area that on the other side of the breezeway door and not in direct line of sight. A re-inspection will take place on 9/12/2023 to ensure the sink has been replaced. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained -------- Observed pooling water along the back wall of the establishment extending from the water heater to the 3 comp sink. PIC stated they had been closed for 3 days and had noticed the water forming. The water appears to be coming from a leak that formed around the water heater base which heavy rusting was observed. Advised PIC to replace water heater or identify and plug leak ASAP. Advised PIC to be self-closed while repair are being made if the water must be turned off. Advised PIC to mop up water as needed to prevent pooling. Observed more than 5 leaks coming from the commercial ice maker into small containers placed around the perimeter of the ice bin. Advised PIC to repair, replace or remove and purchase bagged ice from an approved source prior to the next routine inspection. All runoff from the ice machine must be directed to a floor sink. Core-5-203.13, C: Service Sink-----Observed no mop sink in this establishment. Establishment is currently dumping mop water down the 3 comp sink floor sink. Ensure there is at least 108 lux (10 foot candles) inside walk in coolers. Also observed multiple bulbs throughout the establishment that had burned out and not been replaced. Advised PIC to replaced bulb with one bright enough to illuminate all areas of the walk in to allow for proper cleaning. The manager stated that the bulbs would be replaced as soon as possible.
